Age 75 of Brooklyn Park passed away peacefully surrounded by family & loved ones.
Patricia is preceded in death by her loving husband, Michael Jacobs; father, Victor & mother, Helen Boroski; sister, Janice Skappel.
She is survived by her daughter, Michelle (Dan) Blachfelner, and son, John (Haili) Jacobs; sister, Diane Lindberg; grandchildren, David (Jolene), Michael (Chelsea), Paige (Brad), Madelyn (Tommy), Aiden; great grandchildren, Hazel, Adelynn, and Keith; bonus grandchildren, Natalie, and Brady.
Memorial Service 2:00 PM Sunday, December 7, 2025, at Gearty-Delmore Funeral Chapel, 3888 West Broadway Avenue, Robbinsdale, MN 55422. Visitation one hour prior to service at the funeral home chapel.
Interment at Fort Snelling National Cemetery, Minneapolis.
